Replacement keys

I have been advised that in order to obtain a replacement key for the club (having lost mine), I am to complete a form that is available on the website. 

Does anyone know the whereabouts of that form? It's as lost as the keys to me!

I can't be much help, I'm afraid - but as a website editor I guess I am bound to post some sort of reply!

The form in question may perhaps have been accessible from the old website, but I can find nothing in current resources (i.e. uploaded files going back a while which may or may not currently be accessible via a Downloads link) that answers the description.

It's possible I suppose that the form might have been removed because of incompatibility with the move to WebCollect.

I suggest an enquiry to the Membership Secretary - and I will happily add the form to Downloads once it has been identified, if that is considered useful.
